CONDITIONS FOR ENGAGEMENT

I understand and agree that, if I am engaged, my relationship with Bloomfield Players will be as an independent contractor. No payment of employment taxes or withholdings of any kind will be made. I will provide Bloomfield Players with all information required for any tax or other reporting pertinent to this relationship, including my social security number, if appropriate.

I voluntarily give Bloomfield Players the right to make a thorough investigation of my previous employment, character and educational background. I release from all liabilities any persons, companies or corporations supplying such information.

I authorize Bloomfield Players to conduct a criminal background check, under the auspices of the Bloomfield Hills School District, and will provide the information necessary to do so. I understand that any results of this investigation will be kept strictly confidential and will be available only to the appropriate agents of the School District and to specifically authorized members of the Board of Directors of Bloomfield Players.

I understand that as part of the procedures for processing my application, Bloomfield Players may obtain an investigative consumer report regarding my credit and personal history, in which information may be obtained through personal interviews with third parties acquainted with me. I authorize Bloomfield Players to obtain such a report if it so chooses. If a report is obtained, I understand that Bloomfield Players must provide, upon my written request, the name of the agency so I may obtain from them the nature and substance of the information contained in the report.
